- Are treated for 3 or more medical problems;
- Take medicines 3 or more times a day;
- Have 2 or more physicians who prescribe medicines for you;
- Were hospitalized in the last year;
- Feel rushed at your pharmacy or medical appointments, or still have questions about your medicines.
Consultations are provided by a certified geriatric pharmacist and pharmacotherapy specialist, Meet Dr. Levy.
